Please complete the form and submit it to the centre with the Direct Debit Form and a record of your child’s immunisation.

Direct Debit Request
All families are required to sign a Direct Debit Authority Form and return it to the centre with their enrolment paperwork. Payments can be debited from a bank account or credit card. No CASH or CHEQUES will be accepted. Incomplete enrolment applications cannot be processed.

Asthma Action Plan
To be completed by all families with a child diagnosed with asthma when enrolling.

Anaphylaxis Action Plan
To be completed by all families with a child diagnosed as being anaphylactic. Required at time of enrolment, signed by a doctor. Medication and an Epi Pen will also be required.

Allergy Action Plan
To be completed by all families with a child diagnosed with an allergy. Required at time of enrolment, signed by a doctor. Medication will also be required if this forms part of the treatment plan.
