ASCIA Action Plan for Allergic Reactions
To be completed by all families with a child diagnosed with an allergy. Required at time of enrolment, signed by a doctor. Medication will also be required if this forms states it as part of the treatment plan.

ASCIA Anaphylaxis Action Plan with EpiPen
To be completed by all families with a child diagnosed as being anaphylactic who requires an EpiPen. Required at time of enrolment, signed by a doctor. An Epi Pen will also be required if this forms states it as part of the treatment plan.
